Walden is committed to the health and safety of every patient we serve. If any patient feels that we have not lived up to that commitment we want to hear about it. If your or an immediate family member had a negative care experience with Walden, please feel free to complete the form below. Your submission will be sent directly to our Human Right’s Officers for review. The Human Rights Officer may then refer your submission to our Ethics Committee for further review.
